Get With the Guidelines® – Heart Failure Module
Overview
Get With The Guidelines®– Heart Failure is an in-hospital program for improving care by promoting consistent adherence to the latest scientific treatment guidelines. Numerous published studies demonstrate the program’s success in achieving significant patient outcome improvements. Among the proven results are reductions in 30-day readmissions, a measure now used by CMS in determining CMS reimbursement rates.
Data Submission of Heart Failure Patient Data
Heart failure patient data submission is performed through the American Heart Association’s Patient Management Tool™ (PMT), an online, interactive system provided by Quintiles Real World & Late Phase Research.
This Patient Management Tool is a resource for member Hospital to submit patient data online. However, it’s an extremely burdensome and time consuming process. Your data entry analyst has to scour Patient Record for required data items and then to manually re-enter them into the Patient Management Tool for submission to AHA. This is a long, drawn out process; as the October 2016 Heart Failure data entry form is nine (9) pages long! Now imagine doing this for every one of your heart failure patients? How much time, money and resources will that take?
Lumnacity’s ‘AHA Heart Failure Forms’ to the Rescue
Lumnacity’s ‘AHA Heart Failure Forms’ module will integrate directly with your institutions Electronic Health Record and extract patient data required for submission to AHA Patient Management Tool. Lumnacity’s ‘AHA Heart Failure Forms’ module displays the Patient Information for review to your data entry analyst and allow them to enter or edit the information, if necessary, before moving on to the next patient. Thereafter, our automated Quintiles submission workflow will allow your staff to produce the required and appropriately formatted data file for export to AHA Patient Management Tool automatically, with just one mouse click! This will greatly reduce the time, money and effort required for data submission to AHA’s ‘Get With The Guidelines’ program!
